As they are listet in the code equally (vars(treatment, sex, age, ph.ecog)) it seems to me that the first HR (0.98) is the risk of dying under treatment, the second HR (0.58) the risk of dying if sex = 1, the third HR (1.01) if age60 = 1 and so on.
